The crash happened on Saturday afternoon just west of the Prescott Regional Airport, near Williamson Valley Road and Levie Lane.
PRESCOTT, Ariz. — A father and two of his children have died after a head-on collision in Prescott on Saturday, according to the Yavapai County Sheriff’s Office.
The Central Arizona Fire and Medical Authority told 12News that the crash happened at about 3:01 p.m. on Saturday on Williamson Valley Road near Levie Lane. That area is just west of the Prescott Regional Airport and just east of Granite Mountain.

YCSO said deputies found a father and one child dead at the scene of the crash. A second child died while being transported to a hospital. Three adults were airlifted to hospitals for treatment for their injuries. A third child is in critical condition at a hospital in Phoenix.
YCSO said that the crash happened when a man, identified as Jesse Forman from Chino Valley, was driving on Williamson Valley Road in a Toyota 4-Runner and crossed the center line and collided head-on with a Jeep SUV.
Forman was driving with his three children and their mother in the 4-Runner, while a woman, only identified as being in her 60s, and her son, only identified as an adult, were in the Jeep.
The woman and her son were both airlifted to a hospital in Flagstaff and YCSO says they both have serious injuries.
YCSO did not say how severe the mother’s injuries were.
“The Sheriff’s office extends the deepest sympathy to the children and the families involved in this horrific crash,” Yavapai County Sheriff David Rhodes said in a press release. “I also want to recognize the tireless efforts of the many first responders who immediately rushed to the aid of all involved.”
YCSO said deputies have put extra effort into traffic enforcement on Williamson Valley Road due to “recent complaints from the public.” The sheriff’s office says they will continue to patrol the area where this crash happened and other areas in the county where reckless driving has become a problem.
